Development of a score that predicts Manometric alterations in patients with systemic sclerosis

Macedo Introduction: In patients with systemic sclerosis (SS) esophageal manometry (EM) is generally reserved for patients with symptoms refractory to empirical therapy. However, 30% of patients with abnormal MS are asymptomatic. Our aim was to evaluate the relationship between findings in MS and different clinical and laboratory findings in patients with SSc and to build a predictive model for altered MS. Methods: Retrospective study of 71 SSc patients who underwent EM over a 12-year period (2008-2020). SSc-related EM changes were defined as lower esophageal sphincter hypotension, ineffective motility, or absent contractility. Results: The population studied had a median age of 54 (42-64) years; female in 88%. Diffuse cutaneous SSc in 10.5%, limited cutaneous SSc in 39.5% and overlap syndrome with SSc in 42.1%. Gastrointestinal (GI) symptoms present in 82% of patients (dysphagia in 63%). EM showed changes in 40.8% of patients. In univariate analysis, altered MS presented in relation to Raynaud's syndrome (RR3.6; 95%CI 1.3-5.3; P = 0.024), diffuse cutaneous SSc (RR2.5; 95%CI 1.5-4.5 ; P = 0.007), antinuclear antibody (RR3.6; 95%CI 1.8-4.6, P = 0.034), abnormal cardiac study (RR2.2; 95%CI 1.3-4.7, P = 0.045) and abnormal CO diffusion capacity (RR1.8; 95%CI 1.3-3.2, P = 0.021). Altered MS no presentation with the presence of GI symptoms (RR1.4; 95%CI 0.6-3.4, P = 0.234). We built a model using diffuse cutaneous SSc, antinuclear antibody, cardiac study and CO diffusion capacity, with an area under the curve of 0.748 (P = 0.002). This model has a sensitivity of 86% and a specificity of 74% for EM changes. Conclusions: In our population, gastrointestinal symptoms unrelated to EM changes. Other findings should be considered for changes that are predictive of MS. Our model is able to alter MS prediction in patients with SSc.
